Request for an Appointment

The first step toward a beautiful, healthy smile is making an appointment. We have flexible hours to accommodate most work schedules. On Mondays we are here until 6:00pm while Tuesdays through Thursdays we open 7:00am so you can request a visit before work. Please note your desired appointment time in the "Reason for Your Visit" box. Our appointment coordinator will contact you to confirm your appointment time.
